Fee's Details (non-refundable)
Rs.500/- for GEN/ OBC candidates (While Fee is exempted for SC/ ST/ Ex-Servicemen candidates). The Payment of Fees through Internet Banking/ Debit/ Credit Card, etc. (as provided in the Online Application) will only be accepted.

  • Interested Medical Professionals are required to download the blank biodata form (attached to this advertisement) for engagement of Part-Time Medical Consultant(Specialists), and send their duly filled in biodata form along with self-attested photocopies of the below-mentioned certificates by Post, superscribing the envelope as “Application for PTMC (Specialists) – 01/2020” to

  • The last date for Receipt of Application Form is 15th March 2020.
Documents To Be Sent By Post (Self-Attested Photocopy)
  1. Duly filled in Bio-Data Form along-with two latest passport size photographs).
  2. SSC/ Matric Mark Sheet/ Certificate.
  3. HSC/ Intermediate Mark Sheet/ Certificate.
  4. MBBS Degree Certificate with all Mark sheets.
  5. PG Degree/ Diploma Certificate with all Mark sheets.
  6. Registration Certificate issued by the Medical Council of India or by a State Medical Council.
  7. Any other relevant certificate (if required).
